Lost or Stolen Passport Notification

Use this form to report the loss or theft of a passport. Please write only within the white boxes. IMPORTANT: Completing this form will not provide you with a replacement passport. To replace your passport you will need to submit this form, a passport application form, supporting documents and fee.

Only report the loss or theft of a passport when you have exhausted all efforts. In the Cayman Islands this loss/theft must be reported to the Royal Cayman Islands Police Service (RCIPS). 

Note:

• When a passport is report to the RCIPS it is also sent to INTERPOL and the document is invalid for travel.
• Once the Lost or Stolen form is completed and submitted it is done with the understanding that this will result in:
  • a) the related passport being cancelled, that it may never be used again; and
  • b) if subsequently found, it should be returned to Her Majesty’s Passport Office.
