Consent Letter for Issuance of Passport

A child under 16 must have permission from a person with parental responsibility. If a child’s parents are married, both parents must give permission; one by signing the application at Section 9 and the other in the form of a consent letter, along with a form of identification - Driver’s Licence, Voter Registration ID, etc.
