1. A list of all chemical products applied during this period by both: Aerial spraying operations; and Ground-based spraying operations.

2. For each product used, please provide:

• The product name;
• The active ingredient(s);
• The concentration or formulation used;
• The purpose of the application (e.g. adulticide, larvicide, residual treatment, etc.);
• The dates on which each product was applied;
• The general locations or districts where each application occurred.

3. Copies of any application logs, spray schedules, operational records, or similar documents identifying:

• The dates and times of applications;
• The products used;
• The method of application (aircraft or ground vehicle);
• The geographical areas treated.

4. Copies of any Safety Data Sheets (SDS), product labels, or technical information maintained by the MRCU for each chemical product used during this period, if these are not already publicly available.
